We recognise that transition can be difficult for a child with SEND and we take steps to ensure that any transition is as smooth as possible.
Year 6 Transition - SEN coordinators from secondary schools visit school to discuss the individual learning needs of all pupils with SEND. Current intervention, support and resources being used at Greenbank will be discussed so that a successful transition to secondary school can occur.
Pupils with SEND may be offered additional visits to their secondary school to build and help secure a more successful transition to Key Stage 3. These visits may be supported by a member of staff from Greenbank.
All files and information including reports from other professionals will be transferred between settings to help inform the transfer.
We have regular consultation meetings with our Educational Psychologist, Social, Emotional Behaviour Support team and a member of the Learning Support Services to discuss pupils with SEND. Following this meeting we may ask parents permission to seek further professional support /advice.
Greenbank will use its best endeavours' to meet your child's needs. However, it may be felt following full consultation with you and other professionals working with your child, that an alternative provision is in your child's best interests for now and possibly in the future. If this is the case, then we may consider an alternative placement.
